History

Ankara Hospital of Başkent University was established by Prof. Dr. Mehmet Haberal in 1985 as Organ Transplantation and Burn Foundation Hospital and gained teaching hospital status in 1992 and afterwards accepted first anesthesia specialist student. With the foundation of Başkent University in 1994, Faculty of Medicine and Department of Anesthesiology were established. The founder and the first head of the Department was Prof. Dr. Gülnaz Arslan. Following Ankara Hospital, Izmir Medical and Research Center, Adana Turgut Noyan Medical and Research Center (Yüreğir Seyhan, Kışla Hospitals), Alanya Medical  and Research Center, Konya Medical and Research Center, Istanbul Medical and Research Center were established,  with anesthesiology staff  operating academically under the Department.

Surgical anesthesia, non-operating room anesthesia, anesthesia outpatient clinic, Algology and Intensive Care  services are available in our  department,.

Areas of study and practice

The scope of anesthesia practices has gradually expanded since the first use in the middle of 19th century and there were physicians interested in anesthesia on a full-time basis at the end of the century. First specialist title was given in 1956 in Turkey. While the aim of anesthesia was to relieve surgical pain only at the beginning, today it has changed and expanded tremendously; areas of work outside of surgery and operating rooms are being born. As a result of the developments,  almost all kinds of surgery can be performed and services in many areas like pain treatment, intensive care and reanimation, palliative care, home care can also be provided. The interests of anesthesiology will further expand by taking active roles in pre-and post-operative care of surgical patients as Perioperative Medicine and ERAS approach practices and by contributing to other areas of medicine with experiences gained from these areas.

 

Anesthesia Practices in the Operating Room

Required anesthesia techniques for every kind of surgical processes are available under technologically developed conditions, by our anesthesia team in a total of 54 operating rooms (Ankara-14, Adana-21, Alanya-4, Konya-7, İstanbul-5, İzmir-3). The operating rooms are available for 24 hours a day for all surgical procedures and invasive procedures of the Algology Unit. Procedures are performed both on inpatients and outpatients. Our centers are particularly preferred for complex and specific interventions of high-risk patients. All kinds of analgesia methods are available and used for the treatment of postoperative pain.

 

 

Anesthesia Practices outside Operating Room

In recent years, anesthesia is required both for patients’ and hospital personnels’ security, comfort and wellness, for diagnosis and treatment-oriented invasive procedures in parallel to the developments in medicine and technology. Thus, conscious or deep sedation, regional or general anesthesia are needed for procedures performed out of operating rooms.  Patients are evaluated by anesthesiologist prior to these procedures and consulted with the relevant departments if necessary depending on the presence and degree of accompanying disease. Mainly, invasive radiology, pediatric and adult cardiology, gastroenterology, chest diseases and hemotology/oncology departments, ESWL (extracorporeal shock wave lithotripsy) and dental treatment patients are the ones requiring anesthesia and sedation, during the procedures planned. Patients are monitorized and provided standard care with appropriate and high technology monitors and followed during the procedures, taken the precautions before, by an anesthesiologist. 

Preoperative Evaluation (Anesthesia Clinic)

Patients are evaluated for planning the anesthetic management before surgery by our team.  Organ system functions and laboratory, radiologic examinations are made before surgical procedures and the patients are further consulted by other specialists if needed. patients are informed about the type of anesthesia   and care after anesthesia (discharge, service, intensive care) and postoperative pain control. For preoperative anxiety experienced by the patient, sedative pharmacological or psychological premedication is given.

Other Services

Providing basic and advanced life support services and ensuring continuity of education in these subjects, emergency department, operating room and intensive care unit transfer processes of patients who need life support and mechanical ventilator support treatment are also among the services provided by the Department.

Blue Code alarm system raised by Emergency Unit and as well as all services in the hospital, is also answered by our department providing  basic and advanced life support.

 

Algology

Chronic pain, which reduces the patient's movements, ability to work, ability to sleep and self-care, and capacity to socialize, is exacerbated by the negative consequences if left untreated. For this reason, pain treatment should be planned as effective, disciplined and administered together with the patient. The working principle of the algology clinic is to help relieving the pain of patients with chronic pain and assist their social lives with the proper and sufficient treatment whatever the cause is.

The causes of pain clinic admissions are frequently; lumbar and back pains, head and neck pains, cancer pain, shoulder, arm pains, face pain and neuralgias, and vascular occlusion pain and pains of unknown origin. In algology clinics, a variety of drug treatments or interventional procedures may be applied for chronic pain.

Our unit was founded in 2003 as Pain Clinic, then it became a subspeciality in 2014. In the unit, diagnosis and treatment options are offered to patients with chronic pain by the expert doctors. In necessary circumstances, patients are evaluated with the consultations of other medical branches and the treatment is directed. Interventional processes are performed by using various imaging devices such as ultrasonography, fluoroscopy, computerized tomography, etc. under sterile conditions. During the processes, vital functions are monitored in order to provide patient safety. Sedatives and local anesthetic agents are applied to prevent the discomfort of the patient. Interventions are carried out as day-case procedures and the patients rest for a while after the process before leaving the clinic. The Algology Units at other centers other than Ankara are also active and in service.

Anesthesiology Intensive Care Unit

Units working under different names as intensive care units initially were reorganized as central intensive care services.  In accordance with the Regulation on Expertise Training published in the Official Gazette No. 25.8.2009 / 27292, the specialty training authority was received under the name of Ankara 4th Intensive Care Program. The founder and the first head of the department was Professor Arash Pirat, MD.

In general and specific areas, Ankara (37 beds), Adana (20 beds), Alanya (27 beds), Konya (25 beds), Istanbul (16 beds) and Izmir (6 beds) centers with a total of 131 beds provide intensive care services. Our patient profile consists of adult patients with acute respiratory failure, ARDS (acute respiratory distress syndrome), heart failure, acute kidney injury, acute liver failure, sepsis and septic shock, trauma, liver transplantation, heart transplantation, left or right VAD (ventricular assist device) implantation and high-risk surgical and cardiothoracic postoperative patients.

Our department has various new technological equipment and in addition to standard intensive care treatments; advanced mechanical ventilation strategies, advanced hemodynamic monitoring methods (transthoracic and transesophageal echocardiography, pulmonary artery catheter, PiCCO®, MostCare®), veno-venous and veno-arterial ECMO (extracorporeal membrane oxygenation), extracorporeal CO2 removal, fiberoptic bronchoscopy-guided percutaneous tracheotomy, USG-guided percutaneous tracheotomy and extracorporeal liver support treatments are provided.
